The Director of the Division of Health & Vital Statistics is
the Registrar of Vital Statistics. This Division operates under the
supervision of the Mayor, Administrator and according to City Ordinance
and State regulations. They perform all appropriate functions to
issue and keep records of City of Burlington vital statistics, licensure
of food services, and licensure of pet dogs. The Division activities
include:
- Process Applications and issue City Birth Certificates
- Process Applications and issue City Death Certificates
- Process Applications and issue City Marriage Certificates
- Health Ordinance and Regulations Compliance Review
- Process Food Service Establishment Applications
- Process Applications for City Dog Licenses
- Maintain City Board of Health records
- Conduct City Board of Health Public Meetings

Vital records are kept of all births, deaths and marriages occurring
within the City of Burlington. Contact the Registrar to obtain certified
copies of:
- Birth Records
- Death Certificates
- Marriage Certificates
The cost for a Certified Record is:
$25.00 first copy
$2.00 each additional copy

If you are planning to get married in the City of Burlington, you
will need to contact the Registrar of Vital Statistics. Marriage
licenses must be applied for in the town where the bride lives, unless
her residency is out of state, in which case you must apply in the
town where the marriage is going to take place. Couples must bring
with them:
- Proof of residency
- Social Security numbers
- A witness age 18 or over
- Maiden names of both Mothers
- Name and address of person performing the wedding ceremony
- Address where the wedding will be held
- Divorce papers from previous marriages, if applicable
- Death Certificate from previous marriages, if applicable
Marriage License Application Fee: $28.00.
Blood tests are no longer required. There is a 72-hour waiting period
from date of application to date of issuance, excluding the day of
the wedding.

Proposed food service owners, contact the Registrar to obtain the Application
for Food Establishment Licenses. Fees vary, please call for
further information M–F 9am–5pm 609-386-0200 ext. 137.

The Division of Animal Control does not issue dog licenses.
All dogs living in the City of Burlington over the age of 6 months
must be licensed annually. Cats need not be licensed. Dog licenses are available
from the Health Division / Registrar of Vital Statistics, 9am–5pm
M–F at:

Be prepared to provide:
- certificate of rabies vaccination
- proof of spaying or neutering, if done.
Dog License Fees are:
Dogs spayed or neutered: $8.00/yr.
Dogs NOT spayed or neutered: $11.00/yr.
Dog licenses should be renewed each calendar year during the month
of January! After that, registrations will incur a late fee of
$15.00. The Division of Animal Control does not issue
dog licenses, and Licenses are no longer offered at Keegan Center.

NOTE: The Division of Health and Vital Statistics may not prepare
lists or statistical analyses from City Records.
